1. If you haven’t already signed your lacrosse club or league up with TeamSnap, here’s how to get started.
2. Log into TeamSnap.
3. Enter your club or league under the My Organizations section of the Dashboard screen.
4. Click the Registration tab.
5. To create a new form with the verification fields:

1. 1. Click the +New Registration Form button.
   2. Select US Lacrosse Membership from the template dropdown.
   3. You will see the verification fields under sport info when building out your form.
   4. Continue to complete the rest of your form set up as usual.
